Immigration and Refugee Physicals

We offer Immigration and refugee physicals for all ages here at Family Health Associates. To smoothly assist you we request the additional information at time of scheduling

  • Current government issued Identification, i.e. Passport, Driver’s license, or US Work Authorization
  • Full Name, address and date of birth
  • Place of birth, City/Town and Country
  • Proof of childhood and current vaccinations
  • Medical History and List of Medications

Refugee Physical Requirements

  • Copy of complete I-94 form (Arrival/Departure record).
  • Vaccines required by the CDC for your age range. Please refer to Vaccine Requirements According to Applicant Age.

Immigration Physical Requirements

  • T-Spot TB blood test for Tuberculosis, also known as an interferon gamma release assay (IGRA).  (Including children <5 years of age).
    • Positive tests will require a chest x-ray within 30 days of your immigration physical.
    • If you have history of Tuberculosis treatment, you must provide documented proof prior to your appointment, this includes:
      • Medication name and strength, start and stop date of medication and name of prescribing doctor.
      • If you have a positive history of a PPD skin test you do not need to get another skin test.  You will be required to get a chest x-ray.
      • If you need treatment, you must start the treatment prior to your appointment date.
  • Blood test for Syphilis, treatment required if positive. (If over the age of 14)
    • Test must be done within 30 days of physical.
    • Must show proof of treatment if you have a history of Syphilis. This includes: Medication name and strength, start and stop date of medication and name of prescribing doctor.
  • Urine Test for Gonorrhea, treatment required if positive. (If over the age of 14)
    • Test must be done within 30 days of physical.
  • Vaccines required by the CDC for your age range. Please refer to Vaccine Requirements According to Applicant Age.

All labs, treatment, vaccines, chest x-rays and proof of vaccines are required prior to your appointment.  Once all of this information has been reviewed by our staff, you may set up an appointment for a physical by calling our Immigration Specialist at (541) 567-6434 x 2.
