Family Medical Leave Act

 

 The Family and Medical Leave Act (FMLA) entitles eligible employees who work for covered employers to take unpaid, job-protected leave for specified family and medical reasons, with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave.

FMLA can be requested for the following reasons

  • Recover or seek treatment for your own serious health condition that renders you incapacitated
  • Tend to the serious health condition of your spouse, parent or child
  • Birth or adoption of a child

An employer will require a medical certification containing contact information for the health provider; date of the health condition began and how long it will last; information showing the medical necessity for intermittent or reduced schedule leave.  The employee is responsible for paying for the cost of the medical certification and for making sure the certification is provided to the employer

 

Motor Vehicle Accident Billing Policies

 

If you have been involved in a motor vehicle accident and need to be seen by a medical provider, you will need to provide the following information:

  • Vehicle’s Insurance Card
  • Date of Injury
  • Claim Number
  • Adjuster Name, Address and Phone Number

Please note, if this information is not provided at the time of service the patient will be responsible for the cost of treatment and may be required to pay before being seen.

 

Worker’s Compensation

 

We only accept worker’s compensation claims for existing Family Health Associates patients.

We only accept Oregon Worker’s Compensation claims. We will ask you to provide the standard 801 form, which is the notification to your employer you are filing a worker’s compensation claim. FHA will require the 827 which is the Health Care Provider’s report to be completed in our office upon first visit.

We will not accept Worker’s Compensation claims if they have previously been treated by another provider or if the injury is greater than six months old.

The patient will be responsible for the entire bill if the patient does not inform the front desk at the time of check-in that the services being provided are due to a work injury and the filing time has expired.

 

Third Party Insurance Claims

 

Family Health Associates will not process third party insurance claims.  The patient may be responsible for the cost of treatment at the time of service.  It is the patient’s responsibility to seek reimbursement of medical costs for third party insurance claims.
